The system tried to determine if the disk is an Extensible Firmware Interface (EFI) disk, but it failed to read a particular block. The disk seems to be inaccessible.

Check the status of the disk in the Veritas Volume Manager (VxVM) configuration. Enter:

vxdisk list disk_name

Confirm whether the disk is connected. If it is not connected, do the following:

  1. Reconnect the disk and run the operating system specific commands to discover the disk.
     
  2. Populate the disk information. Enter:

    vxdisk scandisks
     
  3. Retry the operation.
